What does that mean? Not sure! It looks as if the one of the new sound files in your dungeon doesn't contain any data, but if that were the case, I'd expect there to be an error when loading the txt file (as well as quitting the editor). Can you send me the files you're using so I can try and repro...

It's not triggering because there's not an object type for your new item to convert to (specifically, when examining an object you don't want it to convert to nothing - i.e. vanish - so this special case deliberately does not work).

The mechanics to do all of the above already exist. Have a look at the CONVERT_CONSUME and CONVERT_EXAMINE properties of standard objects.
