Have you tried Ultimate Torment and Torture? No doubts the BEST WAD EVER RELEASED (67 MB...). Not sure on wchich engine it runs best though. I`m not sure if it runs on ZDoom, but you may also try Skulltag.

As for DM customs, I recommend Conflux III, Surgicial Strike or The Black Flame (after beating CSB, of course!!!)

I didn't beat the Black Flame... is it cool ?

I confess I tried most old customs, but in many case,
I was stopped by (compose your statement):
dullness/stucked/total confusion/let's play Conflux instead.

Conflux 3 and SurgStrike are really good though.
(but I think CSB is a lot harder than Surgical Strike,
except if you want to find all SurgStrike secrets)

You should give a try to Sukumvit, too.
It's hard, but not too much, well designed,
and there's a lot of original ideas added.
(monsters notably)

And try Dark Portal, too.
It's totally different, but it's one of the best customs.
